I think about this a lot, actually. There are quite a few officers in the Gotei 13 that knew Isshin and/or Kaien; they were one of the noble families after all! And looking at Ichigo, you really can't deny that he is related to them. Just look at them!

Then I remember Jushiro's reaction to seeing Ichigo for the first time and Byakuya essentially saying, "Absolutely not." vs later when he sees the Getsuga Tensho. He had to have been super suspicious by then. Why didn't he say anything? I don't know. But I have a hard time shaking that Byakuya definitely knew.
