Tony and Silvio are having an affair, but they don't think of what they are doing as an affair. They're "taking care of each other", "blowing off steam", or "de-stressing after a difficult situation".
So when Tony talks with Dr Melfi, he does not count Silvio among his affair partners, or mention him at all. He's not an issue Tony feels the need to talk about.
Until he one day does say something that makes Melfi pause, and ask him to clarify who Silvio is, and why Tony's so glad to have him in his life.
And, when Tony tries to explain, he suddenly feels awkward and tongue tied. Not wanting to say too much, just you know "he's a good friend is all"

I will always love the scene in the sopranos where tony tells the lads he's seeing a psychiatrist and it's framed like him coming out
Despite all the violence in The Sopranos, Iโd say this is a definite contender for the most brutal scene on the show ๐ญ
Also, love how Tony at the end can only say "what about Artie" as a counter. He can only think of one guy outside his mob circle who is his friend ๐
