Main guy stuck with a large, noisy, struggling belly as he gradually gains hp for defeated foes, his support class friend (who probably talked him into this) has to deal with his complaining and help him feel better with restoration spells for his stomachache, and using healing hands to rub his big belly

His belly having to work overtime to deal with dungeons bosses, our hero even turning in the quest with the boss still in his stomach. Walking through town, holding back groans as his belly kicks up a fuss as the monster fights inside. Thrusting his belly against counters and/or tables as he makes his way to turn in his mission

Bonus of him having to burp often to burp up the drops that are left in his belly, even thumping a fist against his chest to help him... the better the item, the bigger the burp even
