I think the targetās perception of what their real name is is extremly important in context of bnha. I noticed this a lot when I write posts, if itās clear who Iām writing about I usually use the shortest version of the name (eg. Deku, instead of Izuku or Midoriya), but with Shigaraki Tomura, I mostly use Tomura or Tenko, because as AFO himself said: itās his name.

AFO calles him Tenko before he gives him the name of Shigaraki Tomura, so the name itself is a reflection of the power AFO holds over him.
Obviously one could argue that Tenko was the name given by hisĀ father, who was also abusive, but itās also the name of the family members that he had love for, especially Hana (who unlike the adults is in no way to blame for the child abuse).
But Tenko doesnāt really hold that power anymore, especially given that the abuser behind it is dead, killed by his victim
And AFO seems really proud of himself when he gives (now) Tomura his last name and doesnāt seem to distance himself from it. All For One is his name, his title, his quirk, his general power, but he still is pretty strong about Shigaraki being his name.
If you look at real people with titles, sometimes artistic names or even nicknames, they often use these names in public settings and get refeared to it, but that doesnāt reflect them in private. Anne Hathaway still gets called Anne by most, even tho she made it clear that Annie is how she is called in her private life. Most people couldnāt tell you the real name of Marilyn Monroe or Lady Gaga, but those werenāt the names they used in their private lives. And outside of her family, no one called Queen Elizabeth II, just āElizabethā.
Imo Shigaraki is the name AFO uses to refear to himself in private. Itās a name for himself and his little brother (and the 2.0 verions heās trying to create with Tenko), while AFO is his name for the public, a title for people to fear.
Which would tie in really well with your: ācan we have it both?ā solution, the figure of AFO gets kept in check, but the man Shigaraki, the one who groomed Tenko and made him Shigaraki Tomura isnāt gone until he is called out by his name.
When he steals Star and Stripeās quirk, I wonder if Shigaraki could use it to free himself of AFOās control?
I mean as long as he gets the opportunity, all heād need to say is something like āAFO can no longer control meā or something to that effect, and AFOās biggest victory is suddenly followed by him losing everything he put into Tomura just like that.

My face is having uncontrollable spasms. Great. It hurts really, really, really bad.
I think part of why I have trouble explaining pain to the doctor is when they ask about the pain scale I always think āWell, if someone threw me down a flight of stairs right now or punched me a few times, it would definitely hurt a lot moreā so I end up saying a low number. I was reading an article that said that ā10ā is the most commonly reported number and that is baffling to me. When I woke up from surgery with an 8" incision in my body and I could hardly even speak, I was in the most horrific pain of my life but I said ā6ā because I thought āWell, if you hit me in the stomach, it would be worse.ā
